Registered Nurse Case Manager - RN CM - INTERIM

Qualified candidates will have previous case management experience with exceptional focus on patient care. * Weekly Estimated Pay: $3,300.20

* Weekly Taxable Pay: $2,215.20

* Weekly Housing Stipend: $672

* Weekly Meal Stipend: $413

*Rates may vary based on stipend needs*

ABOUT THE POSITION:

Required Skills:

Traveler must have strong acute care hospital experience.

Previous travel experience highly desirable, as well as utilization review and discharge planning experience.

Nurse will need to be able to do UR, as well as oversee DC Planning

- Certs: BLS (Must be through AHA - for Healthcare Providers)

- Ratio: 1 to 25

- Systems: Meditech, as well as Midas & Interqual for Utilization Review

- Orientation: 4 days Hospital orientation (Tuesday - Friday) & 3 days with Case Management

- Team Model & Roles of the Team: CCA-Care Coordinator Assistant/Secretary: Assists RN by doing all clerical job duties (non-licensed staff). -

Medical Social Worker: Works with the RN by coordinating all aspects of discharging the patient (ordering DME, coordinating services, etc ). Clinical Social Worker: Works with the RN

* Responsible for the coordination of health care decisions by using a systematic approach to ensure treatment plans that improve quality and outcomes

* Responsible for the promotion of cost-effective care within allotted time frame

* Assure payments of hospital-based services meet patient-related utilization of management criteria

* Implementation of safe and appropriate discharge plans

* Asses the Case Management needs of the patient and provide intervention as part of the discharge planning process

* Primary functions include: Clinical Interventions/Discharge, Care Planning Management, Fiscal Manage and Payer/Referral Management

JOB REQUIREMENTS FOR THE RN CM REGISTERED NURSE CASE MANAGER:

* Current licensure as Registered Nurse in state of practice

* Previous experience in case management or related area

* Must be eligible to work in the United States
