Job Description:

*Case Management LVN: *Job Summary:Under supervision of RN, evaluate member assessments and level of care determinations and assist in development of nursing care plans to meet the needs of members and families enrolled in the Health Home Program. Assist families in understanding the implications and complexities of a member's medical condition and care needs. Support members in self-management of health care and progression toward goals. Support members during level of care changes and ensure that discharge needs are met.Education and Experience:* Current California LVN license required.* Three to five years of experience working in a health care setting; managed care or IPA experience preferred.* Experience working with homeless population preferred.* Must possess a valid California driver's license, clean driving record and current insurance.* Bilingual English and Spanish preferredEssential duties and responsibilities:* Work directly with members, their families, and other support givers to ensure coordination and continuity of care according to the needs of the member.* Support member with all transitions in level of care ensuring that discharge information is understood by the member and discharge needs are met.* Tracks barriers and member goals ties and progression toward goals based on member needs.* Ongoing communication with members by phone and in person; documentation of progress.* Works directly with members to increase self-management skills.* Provides ongoing feedback to the other members of the clinical team reporting on progress and program effectiveness.Job knowledge skills and abilities:* Demonstrated knowledge of nursing processes, case management and continuity of care.* Ability to demonstrate respect for the needs of members, support givers, team members, and others and demonstrate excellent customer service.* Willingness to work collaboratively as part of a team all levels of professionals to achieve goals and identify and remove barriers to member health.* Ability to demonstrate a sensitivity to member's social, cultural, language, physical and financial differences.* Ability to work with members and influence behavior through negotiation of care goals and support of member self-management.* Demonstrated knowledge of disease management strategies.* Strong problem-solving skills and ability to identify issues and solutions in administrative and other relevant areas.* Ability to set and re-set priorities based on changes in member situation and needs.* Ability to work independently, organize and prioritize multiple priorities throughout the day.* Strong attention to detail and ability to be accurate, thorough and persistent in solving problems and following tasks through to completion.* Excellent verbal and written communication skills; able to read, write, and speak articulately and communicate effectively with all levels of the organization and member spectrum.* Demonstrated ability to create professional documents and use proper grammar and punctuation.* Ability to be handle all situations in a professional manner and adapt to a changing environment.* Ability to understand and communicate complex health and benefit information.* Proficient in the use of common office technology.* Reliable in attendance and work schedule and adherent to business dress code and good grooming.* Ability to maintain strict confidentiality at all times.Job Type: Full-timePay: From $1.00 per hourBenefits:* Dental insurance* Health insurance* Paid time off* Vision insuranceWeekly schedule:* Monday to FridayAbility to commute/relocate:* Bakersfield, CA 93309: Reliably commute or planning to relocate before starting work (Required)Experience:* Care plans: 1 year (Required)License/Certification:* Driver's License (Required)* LVN (Required)Work Location: One location
