Job Description:

Hoffmann Hospice is searching for a full time Medical Social Worker. Hours are Monday through Friday from 8am-5pm. Occasional evenings and holidays are required.Essential Functions Include:Perform comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plan. Identify the developmental level of patient/family and obstacles to learning or ability to participate in care of patient.Conduct ongoing reassessments of patient/family needs identifying obstacles to compliance and assisting in understanding goals of interventions. Provide counseling as appropriate.Provide short-term crisis intervention and individual or family counseling when indicated. Assists IDT members in recognizing and understanding the social/mental stress and/or disorder that exacerbates symptoms related to the terminal illness. Also to assess for potential risk of suicide and/or abuse or neglect.Identifies support systems available to reduce stress and facilitate coping with end-of-life care.Addresses environmental resources and obstacles to maintaining safety.Assumes the active role of advocate for the patient/family unit.Develops and maintains contact with appropriate community agencies and services in order to promote inter-agency cooperation and to facilitate related referrals, evaluating patient/family response when referred to community agency.by Jobble
