Either schedule is flexible! Pay is $56 per/visit (includes

Licensed Vocational Nurse (LVN) Join a team that brings care home.
We’re looking for a compassionate and reliable LVN to provide
high-quality, patient-centered care in a home health setting. In
this role, you’ll work closely with an RN and interdisciplinary
team to support patients in the comfort of their own homes. What
You’ll Do Deliver skilled nursing care following individualized
care plans Support the RN Case Manager with patient assessments and
care coordination Monitor patient conditions and communicate
updates to the care team Educate patients and families on self-care
and treatment plans Perform clinical procedures within LVN scope of
practice Maintain accurate and timely documentation What We’re
Looking For Current California LVN license Minimum 1 year of recent
nursing experience Current CPR certification Strong communication
and organizational skills Ability to travel within the service area
(valid driver’s license & insurance required) Physically able to
