Please see our moving corporate video, "The Story of Centre for Neuro Skills" at http://www.neuroskills.com/about-us/tour-cns.phpThe Direct Care Provider/Neuro Rehabilitation Specialist (NRS) supervises and directly trains patients with brain injuries with Activities of Daily Living (ADL's), and therapeutic home programs assigned by clinical therapists. Provides skilled instruction and supervision to patients in social, recreational, shopping, and entertainment activities in community settings. Watch this short video to see the vital part you can have in a meaningful recovery!Shifts available: PM (3:00pm - 11:30pm) and Overnight (11:00pm - 7:30am). Weekends required.The company offers a competitive salary and benefits, paid time off, extended sick leave, medical insurance, paid holidays, and 401(k). Continuing education program available to increase wages after only 3 months of employment!Starting wages are $15.00/hr. A continuing education program is available to begin increasing wages and earning bonuses after only 3 months of employment, giving you the potential to earn up to $17.25/hr after just 1 year! Weekly pay!Minimum RequirementsFull time shifts available with rotating days/weekends required. Work may be performed at the residential site, clinic, and/or in the community and includes outside work in ambient temperatures ranging from 30 - 100 degrees F +/- 2 degrees. The residential site may be a home and/or an apartment complex which may have two stories; if two stories, the top story is accessible by one flight of stairs. The Neuro Rehabilitation Specialist position requires a high level of accuracy in terms of charting and documentation in English; dependability for assigned work shifts; ability to be flexible and adaptable to changing needs of administrative staff and patients; ability to communicate clearly with patients with brain injuries and therapeutic staff in English.Teaches, monitors, administers and documents individual patient programs including ADL's (as outlined on the Needs and Services Plans) and therapeutic home programs in English.* ADL's include such tasks such as providing training and assistance with bathing, grooming, hygiene, toileting, dressing, housecleaning, cooking, clothes washing and self-administration of medications. Lifts and carries laundry baskets and grocery bags, which weigh from 2 pounds to over 25 pounds.* Trains patients on self-administration of prescription and nonprescription medications. Follows "7 rights of medication administration". Ensures medications are kept in locked cabinet. May also have additional training on restricted health conditions, i.e. diabetes etc.* Therapeutic home programs consist of physical exercises from Physical Therapy and Occupational Therapy, including walking, swimming in the on-site pool if applicable, vision exercises, math or reading exercises, money management skills and cognitive skills exercises from Speech/Education, which are assigned by clinical therapists to be followed in the Residential setting.* Work is performed in the patients' apartments/residence. At some locations, some apartments may be accessed only by one flight of stairs; thus, requiring repeated walking and stair climbing.Performs bed, bath, toilet and chair transfers, up to 20 times per shift, utilizing approved transfer techniques. Performs proficient minimal assist Bobath transfer for patients up to 150 lbs., and moderate to maximum assistance Bobath transfers with assistance. Alternative transfer techniques include standing, walking, stooping, bending, kneeling, squatting, lifting, carrying, pushing and pulling. Patients requiring moderate to maximum assistance and/or weighing over 150 pounds are lifted/ transferred by two persons. Mechanical lifts may be used as needed.Provides for the safety of patients, visitors, staff and self when interacting with patients who may have behavioral outbursts, which can include yelling, exiting, hitting, kicking, biting, spitting, scratching, and throwing objects.* Recognizes patient escalation, via verbal, physical and visual cues.* De-escalates patient verbally and physically using behavior management techniques according to each patient's program, which may include Pro- ACT (Professional Assault Crisis Training) such as evasive self-defense (ability to get out of the way) and controlling self-defense (restraints).* Physical intervention and restraints include calling for help, lifting, standing, walking, running, stooping, bending, kneeling, squatting, carrying, pulling, pushing and assisting with the restraint of patients weighing more than 200 pounds, with an average weight of 172 pounds.Charts patient activities and progress on programs that occur during the shift, and relays vital information to supervisor by the end of each work shift. Communicates the events of the shift to the next on-duty NRS. Charting requires fine motor manipulation of writing utensils and/or computer, laptop or tablet, and written fluency in English.Supervises and provides skilled instruction to patients with social, recreational, shopping and entertainment activities out of the Residential and Clinic settings. Follows through with established behavior programs, as well as other aspects of each patient's program relating to community outings, budget, etc. Responsible for the safety and well-being of the patient(s) in his/her charge during community outings.Uses durable medical equipment as required by each patient. May clean and maintain such equipment, including but not limited to wheelchairs, walkers, shower chairs, transfer benches, etc. at any given time according to the outline of each patient's program.A Neuro Rehabilitation Specialist may transport patients in company-owned or personal vehicle when company transportation vehicles are unavailable requiring transferring and driving abilities. NRS who transport patients must be a cleared driver, i.e. clear DMV report, registration, and insurance verification.In California, is a Mandated Abuse Reporter. Completes training, including viewing DOJ video, Your Legal Duty, and corresponding materials, etc.Maintains professional and ethical relationships with staff, patients, patient families, and other visitors.Attends staff meetings, in-services, and all scheduled shifts.Comprehends and follows instructions:* Maintain attention and concentration for necessary periods.* Apply common sense understanding and carry out instructions (written, oral or diagrammatic).* Adapt to situations requiring the precise attainment of set limits, tolerances or standards.Performs simple and repetitive tasks:* Ask simple questions or request assistance when needed.* Perform routine, concrete, organized activities.* Remember locations and work procedures.* Make decisions based on simple sensory data.Maintains work pace appropriate to given work load:* Perform activities on schedule, maintain regular attendance, and be punctual within specified tolerance.* Complete normal workday and/or week, perform at a consistent pace without excessive rest periods.Performs complex or varied tasks:* Synthesize, coordinate and analyze data.* Perform job requiring precise attainment of set limits, tolerances or standards.Relates to other people beyond giving and receiving instructions:* Can get along with co-workers or peers without exhibiting behavioral extremes.* Maintain open door policy to encourage active, empathetic listening.* Perform work activities requiring negotiating, instructing, supervising, persuading or speaking with others.* Respond appropriately to criticism from a supervisor and/or other staff member(s).Effectively influences people on a consistent basis:* Convince or redirect others.* Understand the meaning of words and use them effectively.* Interact appropriately with people which includes the provision of appropriate internal and external customer service.Makes generalizations, evaluations or decisions without immediate supervision:* Recognize potential hazards and observe appropriate precautions.* Understand and remember detailed instructions.* Make independent decisions or judgments based on sensory data or known guidelines.Accepts and carries out responsibility for direction, control and planning:* Set realistic goals or make plans independently of others.* Negotiate, instruct or supervise people.* Respond appropriately to changes in the work setting.Performs other duties as assigned.SPECIAL SKILLS UTILIZED, EDUCATION LEVEL, TRAININGHigh School graduate or G.E.D. required. Must be at least 18 years of age.Successful completion of pre-employment, post-offer drug screen, health screen, including TB clearance, and fingerprint clearance.Must attend and complete required orientation upon hire.Successful completion of required NRS modules and tests for training topics which may include but not be limited to the following: documentation, ADL's, Brain Injury Overview, Brain Injury Rehabilitation Principles and Techniques, Employee Handbook/HR policies, Fall Prevention, Medical, CPR/First Aid, Transfer Training, etc.Successful completion of 20 hours of Pro-ACT training first time scheduled after hire.Successful completion of 16 hours of Behavior Tools training.Must successfully demonstrate competency within 3 months of hire on the following: Professionalism, Grocery Shopping, Transfer Techniques, Thick-It, Medications, Skin Care and Incontinence, ILS, Behavior, Client NET and Cooking.Demonstrated ability to safely manage physical loads essential to performance of job.Demonstrated fluency in English sufficient to communicate with patients with brain injuries and therapeutic staff in English, and to complete charts and documentation as described herein in English.Must attend staff development meetings every month for continuing education and training after hire.Must clock in-and-out every shift.Must follow residential dress code.Must maintain CPR, First Aid, and Pro-ACT Certifications.To be a cleared driver, staff must maintain a good driving record under the California DMV or Texas State Department of Public Safety, and submit verification of driving record, registration and insurance.
