***This position will offer hybrid remote/work from home and in-office within the Bakersfield, CA region, with in-town travel up to 60% of the time.The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric full-service management service organization structure. Position Summary:The Community Resource Liaison is responsible for developing relationships and generating referrals with professionals such as physicians, therapists, Hospital ERs, schools, churches and civic groups, area employers, and other healthcare and human service providers.The Community Liaison conducts 4-5 scheduled appointments per day and feels comfortable with cold calling and overcoming obstacles. Electronic documentation of calls and accurate reporting are a critical part of the position.The candidate will also be responsible for creating and working from a strategic plan, planning and attending programs and events, assisting the VP in positioning the products and services favorably in the healthcare market. The Community Liaison is ethical, professional and has excellent customer service.Responsiblities may include:- Identify community resources for members with social determinant needs.- Screen members for needed resources and provide education on available community resources.- Refer members to community resources and follow up to confirm needs are met.- Support Care Coordination and Social Services in meeting member social determinant needs.- Establish and maintain professional contacts in various community agencies that support social needs.- Create and maintain a network of available services for Dignity Health members in need.- Performs other duties as assigned.

Minimum Qualifications:- Two (2) years of experience in a health care, social service environment including any experience conducting community outreach and interpreting patient focused services.- High school diploma required.- Strong interpersonal skills, with an ability to deal with and work well with others.- Strong knowledge of community resources in the local area and with community based health care delivery.Preferred Qualifications:- Bachelor's in Social Work preferred- Previous experience in skilled nursing facility (SNF)
